We are a local home care agency that is growing and expanding and we are in need of a dynamic client care coordinator to provide the best quality of care to our clients in the Treasure Valley area.

As a client care coordinator you would have a variety of responsibilities that range from scheduling and documentation to client pick ups and on call.
Primary Functions Facilitate the process of staffing caregivers with clients by completing the circle the agencys system to ensure that clients are receiving quality care.
Provide supervision guidance and counseling as necessary to staff to maintain standards of care within agency policy and procedure.
Communicate effectively with the scheduling team and office to ensure tasks are being completed in a timely and efficient manner.
Assist clients with a back up plan if a caregiver is unavailable during a scheduled shift Demonstrate understanding and support of agency policies and professional ethics Post and maintain a schedule of vacations leave of absence and time off requests of caregivers Direct and enforce policy and procedure as needed for guidance of employees Provide current accurate ongoing communication and documentation of clientemployee issues Maintain record of any open shifts and measures taken to ensure all clients are being properly attended to Secondary Job Functions Participate in hiring new employees walking them through and explaining the scheduling room at orientation.
Calls to and from clients family caregiver and clients MUST be documented in ClearCare.
Participate in client pickups facilitate and complete paperwork associated with initiating services for a client.
Ensure personal introduction at first time visit to clients home is done with the caregiver.
Introductions are mandatory for Medicaid clients and should always be offered to private pay and VA clients.
Review work schedules time card procedures charting requirements per caseper caregiver Required to do intermittent visits to homes to assure clients needs are being met Supervisory home visits required quarterly to each client Inform administrator of emergency and problem situations Notify agency nurse other staff and Medicaid of significant changes with the client ProvideAssist with caregiver general orientation as well as orientation to clients care plans Requirements Must have a desire and willingness to learn and demonstrate the ability to accept supervision and follow direction.
Must be dependable reliable and responsible.
Must be able to work with others in a professional and ethical manner.
Must be willing to share oncall responsibilities with scheduling partners.
General knowledge of health care services.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
