This position is Part Time; 6-8 hrs per month, and PRN for crisis management
Social Worker Responsibilities to include;
Quarterly Home Visits
Conduct Quarterly Participant Satisfaction Survey's
Assist in Development of BMP's
Assist in Development of Care Plans
Other duties per job description
LSW with 6 months experience in home health, or related field preferred. Preference given to those with experience in Brain Injury or Memory Care.

CPR/First Aid Required
Must have Valid Drivers License (at least 21 years old with clean driving record)
